His nose has been like that since his Bad days. As for the mouth issue, I really think it's just a trick of the lights and shadows. That shirt looks crazy uncomfortable though...

The picture is a shot of his head back, mouth opened wide, while he is singing. The camera angle is giving you a good shot at his open throat and that is all, Nothing strange. I even went to my bathroom mirror and had a look at myself in a similar pose, which by the way made me laugh quite a bit!
